Insurance

We will request to see your insurance card regularly. Please bring your card to every visit and be prepared to present it at our request. Newborns must be added to the policy within 30 days of birth. A birth certificate is required to complete enrollment. Please give this information to your insurance company as soon as possible.  Failure to enroll newborns within the 30 days will result in child not being covered.

Patient Balances

The “patient portion” column on your monthly statement is the amount you owe after we have posted payment from your insurance company and is due upon receipt of your statement. Payments can be made by cash, check, Visa, Mastercard, American Express and Discover. If for any reason, you are having difficulty paying your balance, we are happy to work with you. Please contact Karen in our billing department immediately at 708-364-1550 ex5 to request a payment plan. Failure to make payments on past due balances will result in your account(s) being placed in collections. Once this occurs, we can no longer provide medical services to your family.

Records Transfer

Request for records will incur a copy fee of $25. Record copies can take up to 3 weeks to process. Any request for immunizations records only, are free of charge only if we have the record in our office. Before copying any charts a parent must fill out a records release form and pay the fee.
